Before we begin training, you are required to read and sign a form acknowledging the expectations and potential outcomes of training with regards to your level of participation in the process.

By checking the boxes by the following statements, I am agreeing to the terms & conditions of this training agreement:

I understand that the success of my dog's training depends on my commitment to following through with regular practice consistent with the methods I was taught by Carrie/Good Dog Training.

I acknowledge that if I do not follow the advice and employ the methods and tools discussed during training with Carrie/Good Dog Training, both throughout the training and after sessions have completed, the training will be ineffective.

I understand that no refunds of any kind will be issued for services rendered.

I understand that if I cancel a private lesson within 24 hours of the appointment time, I will be charged a $40 same-day cancellation fee to be paid the day of the cancelled lesson.

I understand that if I cancel Home School training within 72 hours of my dog's first scheduled training session, Good Dog Training LLC will keep my deposit.
